Patient Consent to Testing

As part of our testing process, we ask that all patients provide their written consent to testing. The form also includes additional optional consents related to insurance reimbursement support, research, and education.

Billing & Access

Insurance

CareDx works with most major insurance plans, and many patients have coverage for CareDx testing and services. CareDx tests are covered by Medicare when coverage criteria is met. Commercial coverage may vary. If your doctor orders a CareDx test, the CareDx billing team will verify your benefits, work directly with your insurance, and help with any required preauthorizations or claim reviews.

Billing

CareDx manages the billing process directly with your insurance provider and keeps you informed along the way. Here’s what you can expect after your doctor orders a CareDx test:

  • 1. Your doctor orders the test.

  • 2. Once testing is complete, CareDx bills your insurance and verifies your coverage.

  • 3. Your insurance processes the claim. You may receive an Explanation of Benefits (EOB), which is not a bill.

  • 4. If the claim is denied, CareDx can—with your permission—help file an appeal.

  • 5. If you owe anything out-of-pocket, CareDx will send you an invoice for your remaining responsibility. Do not make payment until you receive this invoice.
