After a kidney transplant, nutrition becomes an important part of recovery and long-term health. Many kidney transplant recipients need to adjust to new food recommendations, medication-related food restrictions, and food safety habits while recovering from surgery, which can sometimes feel overwhelming. Below, find tips on what to eat after kidney transplant, foods to avoid, and more.
Why diet after kidney transplant matters1,2
A kidney transplant diet can help support healing, maintain muscle strength, protect kidney function, support immune health, and reduce the risk of complications. Nutrition recommendations vary based on kidney function, medications, stage of recovery, and individual health needs, but most diet plans encourage balanced eating patterns that include fruits, vegetables, whole grains, lean proteins, healthy fats, and high-fiber foods. Nutrition recommendations are often different during early recovery compared to long-term transplant care.
How should my diet change after a kidney transplant?1,3–9

The first few months
During the first few months after a kidney transplant, nutrition mainly focuses on helping the body recover from surgery, heal wounds, regain strength, maintain muscle mass, and reduce the risk of infection or complications.
Many transplant recipients may need a higher-calorie, higher-protein diet after kidney transplant surgery, because some transplant medications and the procedure itself can increase the body’s nutritional needs. Your care team may also monitor nutrients and electrolytes such as potassium, phosphorus, and magnesium and recommend dietary adjustments if needed.

In the long-term
As recovery progresses, your kidney transplant diet plan may change over time. Long-term diet guidelines are often based on: kidney function, medications, blood pressure, heart health, cholesterol levels, diabetes risk, and overall health status.
Long-term nutrition goals focus on supporting overall health while helping protect the transplanted kidney. Many patients are encouraged to eat more fruits, vegetables, whole grains, and other high-fiber foods while limiting ultra-processed foods, saturated fats, trans fats, and added sugars.

Many kidney transplant recipients are encouraged to focus on balanced, nutrient-rich foods during recovery and long-term care. Foods may include:
Healthier cooking methods, such as baking, grilling, steaming, and roasting, are generally recommended over frying. Processed meats and heavily fried foods are often limited because they can be high in sodium and saturated fat.
Diet recommendations can vary after transplant. Some patients may need to limit or monitor certain foods—including those high in potassium, phosphorus, or other nutrients—depending on kidney function, medications, and individual health needs. Always follow the guidance of your transplant team or registered dietitian when making dietary choices after transplant.

Foods to avoid after a kidney transplant2,4
Some foods may need to be avoided after a kidney transplant because they can interact with anti-rejection medications (immunosuppressants) or increase the risk of foodborne illness.
Fruits such as grapefruit, pomelo, pomegranate, star fruit, and Seville oranges may interfere with certain transplant medications and should generally be avoided. Consult with your transplant team on what is right for you.
Foods that may increase the risk of illness after kidney transplant include:
Raw or undercooked meat
Sushi, raw seafood, and shellfish
Raw or undercooked eggs
Unpasteurized milk or cheese
Raw sprouts, including bean and alfalfa sprouts
Spoiled or expired food
Your transplant team may also recommend avoiding high-risk food settings, such as buffets, salad bars, or foods that have been sitting out for extended periods, as these may increase the risk of foodborne illness.

Food safety is an important part of life after a kidney transplant because it can help lower the risk of foodborne illness and infection. Anti-rejection medications (immunosuppressants) can weaken the immune system, making it easier to get sick from unsafe food.

How much should a kidney transplant recipient drink?2
Getting enough fluids after a kidney transplant is important for supporting kidney function and helping prevent dehydration. Most kidney transplant recipients are encouraged to drink about 1–2 quarts (32–64 fl oz, or approximately 1-2 liters) of fluids each day. Your transplant team can help guide you.
Water is typically the best choice for hydration. High-sugar beverages and energy drinks should generally be limited because they may not support long-term kidney and overall health.

How much salt should a kidney transplant recipient have in their diet?
Most kidney transplant recipients are encouraged to limit sodium (salt) intake to help maintain healthy blood pressure levels and reduce water retention. In general, sodium intake is often limited to no more than 3,000 mg (3 grams) per day, unless your transplant team recommends otherwise.
It may also help to avoid adding extra salt to foods and limit foods that are high in sodium, including:
Processed meats
Canned soups
Fast food
Pickles
Highly processed packaged foods
